The Client Consultation Form was originally designed by
a qualified practitioner for use in his own clinic. It
is based around the traditional '10 questions' and is
arranged so that its use in Clinic follows a natural progression.
It is as much a mnemonical aid for the practitioner as
it is a template for patient record. It will be unnecessary
in practice to fill all these fields, and those spaces
left vacant on the sheet are best used for listing the
acupuncture points and the herbal formula that has been
applied. It is kept to two sides of A4, such that a single
sheet printed on both sides caters to the requirements
of a typical initial consultation.
Most practitioners will want to tweak this template to suit their
own preferences and treatment style, but if you have any suggestions
